The MLCSU Personalised Healthcare Commissioning (PHC) Team based in Leicestershire are committed to ensuring excellent healthcare for all individuals.
We have a vacancy within the Leicester, Leicestershire and Rutland Review Team for a Learning Disabilities nurse or Mental Health nurse with experience of Continuing Health care who can work autonomously and collaboratively in a multi-agency environment with individuals and their representatives to support the assessment and review process for NHS Continuing Healthcare (CHC) and NHS Funded Nursing Care (FNC). You will undertake comprehensive person-centred assessments to CHC and FNC eligibility.
You will experience of working with Learning Disabilities individuals
You will have a good understanding of the National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care, excellent communication and problem-solving skills, have a proven ability to produce high quality documentation whilst working to deadlines and targets.

To assess, review and actively case manage care home with nursing clients and individuals with Learning Disability / Mental Health needs, within their own homes / supported living who are considered eligible for NHS Continuing Care and funded nursing care contribution (FNC).
To work closely with local partners, to include health, social care and independent providers in the planning, management and review of patient care needs. Provide clinical advice in relation to NHS Continuing Care and NHS Funded Nursing Care (FNC).
To progress the delivery of care under the Personal Health Budget agenda
To liaise with internal and external organisations and lead on the progression of Transition cases through the Continuing Healthcare process

Key Responsibilities
Working within a team of Personalised Healthcare Commissioning Assessors, taking a lead role in complex cases, ensuring a high-quality service is delivered, supporting and mentoring staff to achieve continuous improvement
Managing a caseload of patients and contributing to performance improvement, taking a lead for identified areas, where agreed
Managing a defined caseload for those with complex health needs
Managing staff in line with regional and local HR policies
Facilitating and contributing to multi-disciplinary assessments and patient reviews in line with standard operating procedures
Active involvement in the recruitment of junior members of staff
Supporting the Personalised Healthcare Commissioning team to ensure workstream plans are fulfilled and supervised effectively to ensure successful delivery
Supporting the Clinical Team Leader to ensure that all processes implemented are compliant with the National Framework for NHS Continuing Healthcare and Funded Nursing Care.
Deputise, as required, in the absence of a Clinical Team Leader for Personalised Healthcare Commissioning
Assisting the Team Leader for Personalised Healthcare Commissioning with resolution of complaints, appeals and disputes
Raising issues of concern through the Senior Management Team where necessary
Mentoring and supervision of Personalised Healthcare Commissioning Practitioners and students.
Contributing to performance improvement within the team of Senior Practitioners, Practitioners and Associate Practitioners, ensuring services are delivered to a high standard
Monitoring workstreams of Personalised Healthcare Commissioning Practitioners and quality assurance of processes
Working with members of the team to investigate the causes of any variance from target/plan and proactively contribute to the implementation of solutions
Actively supporting the Personalised Healthcare Commissioning team in working towards goals and objectives
Communicating information, risks, issues and dependencies, including briefings and reports
Maintaining professional registration and up-to date knowledge of legislation
Providing relevant and timely specialist advice and guidance on functional and information matters relating to the service
Delivering against objectives, achieving quality outcomes, prioritising own workload and working to tight deadlines
Fostering transparency and a person-centered approach with patients, families and representatives
Promotion of the use of Personal Health Budgets for eligible individuals as a means of having control over their care and achieving best possible outcomes whilst being cost effective and value for money
Ensure the principles and duties of safeguarding are holistically and consistently applied
Contribute to maintenance and monitoring of service standards around quality
Comply with NHS Codes of Conduct and Standard Business Conduct and/or NHS Trust Standards of Business Conduct policies
