Community Assessment Service:

A team designed to relieve the rising pressure on the NHS and Social Care across Wales. A team that has grown from strength to strength over the past 5 years.

You would part of the Community Assessment Service - Your job is to support individuals from hospital/institutionalised care, back home into the community. During this you will assess the needs of the individual over a period of 6 weeks then complete a person-centred assessment/care plan to suit their needs. You will work closely as part of a team of social workers, hospitals, external support services, to ensure that the service users final care package Is best suited to them.

13.06 for unqualified QCF

14.30 for Level 3 QCF.

45p a mile

Rolling rota - 3 days one week 4 days the next, Every other weekend off.

10 hour shifts - mornings and evenings.

Area's include - Llanelli and surrounding areas.
