The Hospital Assessment & Discharge Team undertakes initial assessments, reviews and safeguarding enquiries for vulnerable people and their carers in the hospital. We work with adults over the age of 18 with physical and sensory disabilities and mental health needs.
To provide an excellent customer focused service we use a high-quality, strengths-based approach. This minimises the distress experienced by service users and ensures good outcomes are achieved.
As part of the services we provide, some service users are identified as requiring additional therapy to support them to regain their previous level of independence or a short stay in a Recovery Bed, again to enable them to regain some level of independence.

We understand that working within social care isn't an easy job and how important it is to take care of ourselves.
Working in Huddersfield you will be responsible for supporting individuals to identify their ongoing needs to ensure a safe and effective discharge.
You will be required to arrange to meet with the service user, and their family members/carers to identify what services are required to meet the service users care needs. You will be required to complete a person-centred assessment and to source relevant services.
You will also be required to attend multi-disciplinary meetings.
You will facilitate safe discharges, with appropriate support from Deputy Team Managers.
At times of the year, the Assessment and Discharge Teams experience severe pressure due to increased numbers of attendance and admission. At these times you may be required to support the teams to ensure that service users who have been deemed medically fit are discharged in a safe and timely manner from Hospital.
In order to be successful in this role you will:
Have experience of working in a social care environment including offering Information, advice and guidance to people.
Have an awareness of relevant legislation pertaining to Adult Social Care.
Have an understanding of the differing requirements of people with a wide range of communication and other complex needs.
Have a standard of literacy and numeracy to be able to complete financial assessments, keep up-to-date records, write assessment, support plans and reports, correspondence and collate information.
Have an awareness of a range of equality issues and their impact on service delivery.
Work as a member of a team, in a multi-disciplinary setting and build strong professional relationships.
Travel is an essential part of this job and therefore it would be extremely difficult for you to do your job effectively if you did not use a car for all or most of the time. It is essential that you hold a full and valid driving licence and it is expected that you will either use a council vehicle or your own car. (Exceptions may be made for disabled applicants).
