Key Responsibilities
Provide practical and emotional support to community members, promoting independence and dignity
Assist with
daily activities

Light personal support
Mobility assistance
Meal support
Support the development and delivery of
individualised care and support plans
in collaboration with the care team
Maintain
accurate written records
of interactions and support provided
Communicate effectively with:
Service users
Families
Healthcare and community professionals
Support individuals with
appointments, errands, and community outings
