Job Description

This is an exciting opportunity for 5 motivated, enthusiastic and organised Community Care Officers to join an energetic and dynamic Urgent and Emergency Care Team, working across Nottinghamshire South covering Rushcliffe, Broxtowe and Gedling locality areas.


We are a team of Social Workers and Community Care Officers who are committed to ensuring that people are discharged from hospital safely, with the right support and ensuring any ongoing rehabilitation needs are identified to enable people to live as independently as possible.


We form part of the Acute/sub-acute discharge services in South Notts, supporting discharges out of Nottingham University Hospitals and Lings Bar rehabilitation Hospital.


You will be required to work flexibly during times of need, and this may include weekend working on occasions.


You will be required to commission services and monitor packages of care within set timescales. You must have knowledge of community care legislation such as The Care Act 2014, Mental Capacity Act 2005, Carers Act, etc and relevant policies and procedures and be committed to providing a high-quality service. Ongoing training is provided and is an essential aspect of this role.


You will contribute to the strategic development of the service including Strengths Based Approaches, proportionate assessments and outcome focused personal budgets. We work closely with our colleagues in Health to promote holistic assessments of need, and jointly with people and their Carers and Families to foster a partnership way of working within a Strengths Based Approach.


The ability to communicate verbally and in writing with people receiving our services, carers and other professional colleagues is essential.


This post carries a casual user car allowance or taxi service for disabled employees.


Ability to work some weekends and some bank holidays is a requirement of this post.


Criteria for application form:

At least two years' experience of work gained through paid employment or extensive personal experience or voluntary work Knowledge of community care services within a health or social care setting and knowledge of the legislation and policies in relation to adult community care services. Experience of independently managing and prioritising demands and tasks to meet objectives. Experience of keeping detailed records and constructing reports or formal letters/submissions Experience of operating as part of a team and assisting others in their work Demonstrate experience of using information technology in a range of applications.

Full driving licence and access to a car unless disability precludes this.
