Full-Time / Part- time: Full time 35 hours per week. may consider part time or job share

The Community Connector will play a key role within the wider Integration of Health and Social Care Agenda. The core aim is to support people to live well through strengthening connections between community resources and primary care. They will provide a responsive person-centred service aligned to a GP Practice team within East Ayrshire.

The Community Connector will be expected to:

Provide a specialist support service to patients, who are often experiencing complex social circumstances. Negotiate and support access for patients to community-based services and activities that will support them to live well. Build effective pathways to support patients to navigate existing complex circumstances. Maintain accurate records to allow effective monitoring and evaluation of interventions. Work autonomously to manage workload and deliver reports in an agreed format timeously. Enable and support primary care staff to build and maintain a comprehensive knowledge of local community services and activities which can be utilised to the best interest of patients. Undertake administrative tasks, maintain effective recording systems and respond to queries. Work to model CVO East Ayrshire vision, values and social objects within all areas of work Attend line management supervision, staff meetings and team events. To undertake relevant training to improve competence and confidence in performing the role. Occasionally carry out additional duties in conjunction with CVO East Ayrshires charitable objectives.
Person Specification:

Knowledge and experience of working with people who are experiencing complex social and emotional circumstances. For example, people who have social, emotional and mental health issues, live in complex life circumstances, experience chronic pain, drug and alcohol misuse or homelessness.

Strong understanding of the challenges faced by people living in areas of deprivation in relation to living well.

Proven and highly effective interpersonal and communication skills in working with people on a 1:1 basis and with cross sector organisations.

Excellent negotiation and motivational skills in order to engage patients to enable them to participate in a wide range of community services and activities.

Skills and Attributes:

A solution-focused approach with non-discriminatory values.

Excellent networking and information management skills.

Ability to work effectively as part of team and on own initiative and willingness to undertake any training as directed.

IT skills in word processing, spreadsheets, email, internet and social media.
