Champion community insight and lived experience in PCN planning and service design. Promote digital inclusion and ensure equitable access to information and support. 2. Personalised Care and Empowerment Provide holistic, person-centred support for patients referred to by PCN practices and partners.
Use coaching and motivational interviewing techniques to help individuals identify personal goals and connect to suitable support. Empower people to build confidence, resilience, and self-management skills. Promote social prescribing and personalised care approaches within PCN. 3.
Collaboration and Integration Work collaboratively with ARRS colleagues Care Coordinators, Health & Wellbeing Coaches, Pharmacists, etc. to deliver proactive, integrated care. Act as a connector between practices, INTs, and community partners. Strengthen partnerships with the ICB, local authorities, and voluntary organisations.
Support the development of PCN hubs and neighbourhood working models to deliver care closer to home. 4. Data, Insight and Evaluation Record activity and outcomes using PCN-approved systems e.g., EMIS / Elemental. Evaluate community initiatives and produce impact reports demonstrating improvements in wellbeing and social value.
Use population health data and community intelligence to inform proactive care planning.
