Requirements:

Complete the mandatory HEE e-learning programme, found at: HEE e-learning for healthcare Enrol in, undertake or qualify from appropriate training as defined by the Personalised Care Institute PCI Responsibilities 1. Community Development and Capacity Building Map, connect, and strengthen community assets and local networks across the PCN footprint. Identify gaps in provision and co-design new projects with residents, VCS partners, and Integrated Neighbourhood Teams INTs. Support voluntary and community sector groups to access funding, resources, and sustainability opportunities.



Champion community insight and lived experience in PCN planning and service design. Promote digital inclusion and ensure equitable access to information and support. 2. Personalised Care and Empowerment Provide holistic, person-centred support for patients referred to by PCN practices and partners.



Use coaching and motivational interviewing techniques to help individuals identify personal goals and connect to suitable support. Empower people to build confidence, resilience, and self-management skills. Promote social prescribing and personalised care approaches within PCN. 3.



Collaboration and Integration Work collaboratively with ARRS colleagues Care Coordinators, Health & Wellbeing Coaches, Pharmacists, etc. to deliver proactive, integrated care. Act as a connector between practices, INTs, and community partners. Strengthen partnerships with the ICB, local authorities, and voluntary organisations.



Support the development of PCN hubs and neighbourhood working models to deliver care closer to home. 4. Data, Insight and Evaluation Record activity and outcomes using PCN-approved systems e.g., EMIS / Elemental. Evaluate community initiatives and produce impact reports demonstrating improvements in wellbeing and social value.



Use population health data and community intelligence to inform proactive care planning. Expected Outcomes: Stronger, better-connected communities across the PCN footprint Increased access to non-clinical, community-led support Measurable improvements in wellbeing, activation, and participation Enhanced partnership working between primary care and the community sector Reduction in health inequalities and greater sustainability in local care models
