As a Community Health and Wellbeing Worker (CHWW), you will be deeply engaged with your local community, using your knowledge to make a meaningful impact. The outreach efforts of a CHWW are crucial in addressing growing health and social care inequalities, while also identifying unmet needs within the community.

This role offers an exciting opportunity to create positive change in your area through an innovative approach, driven by your passion and interpersonal skills, to support selected households. It will involve building long-term, trusted relationships with a diverse range of households, regardless of age.

Your role will focus on fostering consistent relationships aimed at enhancing the overall health and wellbeing of the community. As a CHWW, you will serve as a vital link between the community and health and care services, working to improve access to essential services that meet the needs of those you serve.

Main duties include:



Monthly household visits (or more frequent, depending on the case) within a geographical area (defined as a micro-area). The visit entails building relationships and providing relevant knowledge to individual householders as identified. This is a proactive and integrated approach.

Relationships



Develop meaningful relationships within the local community including the Community Hubs Getting to know the wider support services eg housing, education and employment at regular training sessions Have regular contact with the local social prescribing team and the local GP practices Build close relationships with the CHWWs from the other sites across the integrated care area via monthly training sessions

Support



Empathetic listening ear to each householder Adopt a trauma informed approach to supporting each householder (through training) Adopt low level health coaching and motivational interviewing approaches including goal setting as necessary (following training)

Navigational



Support households to navigate the community and statutory services appropriate to their needs Signpost and refer into to existing services as necessary including housing, education, employment and financial support

Community engagement



Identify local community assets and promote a community-based prevention working closely with the local social prescribing team Identify and advocate for the needs of individuals and the community by sharing this information with your multi-disciplinary team

Educational



Provide lifestyle advice such as smoking cessation, alcohol consumption, healthy diet, and physical exercise as necessary (following training)

Health awareness



As directed by the local GP practice, prompt householders with regards to immunisation, health screening appointments and general appointments

Data collection



Complete notes on each household visit on a template (paper and electronic) Record activity into the relevant computer database Contribute your work and findings to the local multidisciplinary team as necessary.

The post holder must maintain confidentiality, security, and integrity of information relating to people during the course of the duty.

Data protection:

The post holder must be aware of their obligations in respect of the Data Protection Act 2018.

Safety:

The post holder must be aware of their role in safeguarding and promoting the welfare of children and adults. They must also be aware of their responsibility in respect of lone working policies.

Qualifications



The knowledge and skills listed below may be acquired through various types of education, training, or experience.

Minimum requirements:



Basic computer skills

Desirable additional requirements



Community based experience providing advocacy and support which has included public contact Volunteer experience or other community experience

Knowledge, skills and qualities

Desirable

Knowledge of basic health promotion and protection Knowledge of the NHS and broader services Basic understanding of child development Knowledge of local languages
