The role is primarily to support the provision of Health & Social Care of Older Adults resident in Merthyr Tydfil, who are experiencing mental health issues within the context of a multi-disciplinary, multi-agency team and support the delivery of evidence-based mental health care to individuals within a community setting.
The post holder will deliver specific components of the individuals' care plans are directed by the Registered Nurse and/or care coordinator.
The post holder will be required to work without direct supervision for all or most of their shift.
The post holder will be expected to assist with the organisation and facilitation of monitoring the physical and mental health needs and may run or assist with therapeutic groups.
Working within a multi-disciplinary team under the direction of the Team Leader, RMNs and/or Care coordinators.
Provide support to individuals and/or their care partner(s) on an individual and/or group basis by participating in the delivery of care and treatment planning both within the Community setting.
Ensure that individuals engage effectively with the agreed management plans and access appropriate services provided on a regular and consistent basis.
With support from the Community Team leaders and the responsible RMN/Care coordinator, be responsible for and manage their own workload and safety.
Actively participate in continuing professional development through the Knowledge Skills Framework.
Participate in caseload management supervision from the Team leader and/or responsible RMN.
