As a pharmacist member of the Medicines Management Team, the post holder will help develop medicines related procedures, provide training for nursing colleagues on safe handling of medicines, undertake clinical audit and investigate medicines related patient safety incidents.
In addition, the post holder will have a responsibility for providing clinical pharmaceutical support to the Trust's intermediate care beds, supporting the ward-based clinical pharmacist. Ideally the post holder will be a prescriber, or be willing to undertake training to become a prescriber
To support the ward-based clinical pharmacist, providing pharmaceutical carer for the Trust's intermediate care beds
To promote and monitor safe evidence-based prescribing, handling and administration of medicines
To investigate medicines related incidents and disseminate learning
To provide services with training and evidenced based medicines information
To lead on specialist areas of medicines management, protocols and audit as specified in yearly objectives and at management supervision
