Location: Camborne, Cornwall (Hybrid - community / office / home-based)

Contract: Fixed term until 7th January 2027 (funding confirmed until this date; possible extension)

Hours: full time or part time

Salary: 25,798 FTE (pro rata for part time hours)

Transformation CPR delivers community centred services in Cornwall, working closely with Volunteer Cornwall and local health partners to reduce inequalities and improve lives in the area's most disadvantaged communities.

We're proud to run the Community Health and Wellbeing Worker (CHWW) programme, which provides personalised, non medical support to help residents live healthier, more connected lives.

The role



As a Community Health and Wellbeing Worker, you'll build trusted, long term relationships with households in your local area. You'll help people access services, connect with community activities, and take practical steps towards better health and wellbeing.

This is a visible, community facing role requiring regular outreach and accurate record keeping. You'll be supported through induction and training to develop confidence in your work.

Key responsibilities



Engage with residents through visits, calls and outreach to build trust and identify needs. This will include door knocking throughout the year.

Support people to navigate local services and make appropriate referrals.

Use a trauma informed and coaching approach to encourage self management and positive change.

Record all activity accurately in line with GDPR.

Work closely with GP practices, social prescribing teams and other partners.

Promote local health initiatives such as screenings and immunisations.

Take part in regular supervision, training and team meetings.

Person specification



Essential criteria



An understanding of issues faced by households in deprived communities, safeguarding responsibilities and confidentiality.

Be compassionate, non judgemental and able to build rapport with a wide range of people.

Able to work in various environments and have the ability to adapt to change.

Strong interpersonal and listening skills; organised and reliable.

Competent IT and record-keeping skills.

GCSE (or equivalent) in English and Maths.

Willingness to undertake training.

A passion for Camborne and it's residents!

Desirable criteria



Previous community, outreach or support work experience.

Knowledge of local health and voluntary sector services.
