Above everything else, our service is patient centred.The role is predominantly office based and will provide comprehensive administrative and clinical support to the CPCT Hub 5 days a week. It may also include deployment to support outpatient clinics, and urgent community visits when required, and provide ongoing support for the stable patient caseload.Main duties of the jobThe post holder will work without direct supervision to facilitate and coordinate daily patient flow within the Community Palliative Care Team (CPCT) Hub.
