This is an exciting opportunity for a Registered Nurse Learning Disabilities to join Community Services in the Learning Disability Division in Cardiff and the Vale, hosted by Swansea Bay UHB.As a Registered Nurse you will be leading on the delivery of the health priorities for people with a learning disability with a particular focus on improving people's access to annual health checks and health action plans via their GP services.You will have an understanding of the knowledge and skills of the professional activities within a Community Support Team within the field of Learning Disabilities.You will be responsible for managing a clinical and case management caseload, and will liaise with both Social & Healthcare professionals to meet client need. You will support less senior members of the team, mentoring and being exemplary role models for high standards of care.Contribute to the development, maintenance and evaluation of systems and structures, which promote the rights, responsibilities and diversities of people with a learning disability, through effective communication.The post holders will promote the health and wellbeing of clients by providing specialist nursing assessment and advice and training to clients, carers and other professionals.Main duties of the jobWe are looking to recruit a Nurse that can demonstrate our values and welcome applications from people who share Our Values: caring for each other, working together and always improving.If you think you can demonstrate our values, and: -

  • Demonstrate an understanding of the Welsh Government Learning Disability Strategic action plan with a particular reference to increasing the uptake of annual health checks.

  • Develop a timetabled plan alongside colleagues in Primary Care to deliver the outcomes identified.

  • Evidence of undertaking presentation to groups and demostrating teaching and assessing skills. Excellent written and oral communication skills.
  • Ability to prioritise workload amongst competing demands and resolving/addressing complex situations where conflict may be a factor.
  • Demonstrate knowledge of Health Board and National Policy frameworks, funding issues e.g. CHC, relevant legislation e.g. MHA, MCA, MHM.
  • Demonstrate evidence of ability to work on own initiative organise self to work efficiently and effectively.

  • Welsh Language Skills are desirable levels 1 to 5 in understanding, speaking, reading, and writing in Welsh

  • RNLD Current NMC Registration
  • Evidence of Post Registration Education to Degree level or working towards.

  • Experience/knowledge of risk management and working with families.

  • Detailed knowledge of supervision systems and evidence of clinical leadership
