Hours: 37.5 hrs per week



Salary: 25,070 - 26,974 - 12.85 - 13.83 (Salary Point 19-23)



whilst this is a scale for progression, the starting salary is point 19*

About the Role/Secondment



The Community Link Worker will provide person-centred support to people in response to the needs and interests of those attending the Wellbeing Hub, supporting them to identify and manage issues and barriers that affect their ability to live well. You will utilise social inclusion and community mapping approaches to develop links and relationships with other agencies and create opportunities for individuals to achieve their personal goals.

The Community Link worker will be based within the Wellbeing Hubs and potentially community settings/GP practices and will work as part of a MDT which includes psychology, occupational therapy and mental health nursing.

The Community Link Worker will also deliver psychoeducational groups. This role will be From Monday - Friday.

What we are looking for



We are looking for candidates who have experience in completing structured assessments, working with people experiencing social and emotional circumstances, including people with mental health issues and experience in developing and establishing networks with external services and agencies.

Ideally, candidates will have SVQ level 2 or relevant experience in health and social care. You should be able to plan and prioritise your workload and communicate effectively with statutory and non-statutory agencies.
