We are looking for a Senior Nurse, experienced working with patients in the community with complex long-term conditions.

Covering a secondment you will join with our North Kerrier Community Matrons/Long-term Conditions Nurses who, as part of the wider Community Nursing Team, work closely across Adult Community Services, Adult Social Care and Primary Care, using enhanced clinical skills to enable patients to remain independent at home for as long as possible and with the aim of reducing the risk of avoidable hospital admissions.

This is an exciting opportunity as you will be expected to work in partnership with others to develop, take forward and evaluate direction, policies and strategiesas we seek to expand our proactive frailty approach within the North Kerrier Neighbourhood Team.

Main duties of the job

Autonomous Clinical Practice:

Ability to take a comprehensive history, perform a systematic physical examination and establish baseline data to inform the development of a comprehensive and personalised care plan.

Integrate care across all care settings; preventing duplication, fragmentation and delay occurring as patients move between care settings

Establish the dynamic process of understanding, reflecting, active listening and checking understanding, thus developing a therapeutic relationship with patients and their carers.

Education:

Provide the knowledge, and utilise coaching to empower patients and families to make informed decisions and participate in care planning.

Play an active role in disseminating knowledge about chronic disease and its management within the Community Nursing Team, mentoring students as required and developing education for other colleagues.

Leadership/Management:

Contribute to the development of policies, procedures and guidelines, ensuring they are implemented and monitored.

Research:

Develop at least one special area of expertise to be seen as a local expert, able to articulate the most contemporary evidence, and approaches to practice and management.

Working for our organisation

We're an NHS community and mental health provider Trust based in Cornwall and the Isles of Scilly. We deliver community and hospital-based care to improve people's physical and mental health. We also provide specialist support to people with dementia or a learning disability.

We are a people organisation and people matter to us. As part of the team, you'll help support the health and wellbeing of the people who live and visit this beautiful part of the UK.

Over 4,000 people make up the Trust. This includes doctors, nurses, therapists, plus admin and support staff.

We work in people's homes, in community clinics and bases. Some staff work from one of our 13 community hospitals. Our aspiration is to have great people, provide great care, be a great place to work and a great partner.

Approximately 568,000 people live here. A third of people who live in Cornwall are supported by acute hospital services in Devon. As a result, we also work closely with our partners in Devon. In the summer, and during other holidays lots of people choose to visit the area. This increase the numbers of people who use out services.
