Job Description

Key Responsibilities Clinical Care Provide advanced clinical assessment, diagnosis, and management of adults with complex, long-term conditions including frail, elderly, and housebound patients in their homes, care homes, and community healthcare settings. Deliver proactive clinical care, including falls risk assessment, polypharmacy review, and anticipatory care planning. Work closely with families, carers, and social care teams to support patients with complex needs. Leadership & Coordination Lead and coordinate clinical care pathways with GPs, community nursing, care homes, therapy teams, social care, and voluntary sector partners.

Support the development of proactive care initiatives and early intervention within the practice and PCN. Full details of all responsibilities are provided in the . Experience in supporting older adults including frailty, geriatrics, care homes, or complex long-term conditions is highly valued, while also recognising the broader needs of adults of working age.
