As a
Healthcare Assistant (HCA)
, you will work closely with our
multi-disciplinary team (MDT)
, which includes a Consultant Psychiatrist, Community Psychiatric Nurses (CPNs), Memory Assessment Nurse, Occupational Therapist, Dementia Liaison Nurse, Psychologist, and the Primary Care Dementia Team.
What we're looking for:
A caring, compassionate approach and excellent communication skills.
Ability to work as part of a team and independently in the community.
Previous experience in healthcare or mental health support is desirable but not essential - full training will be provided.
Main duties of the job
What you'll do:
Support patients and their carers in the community, either in their own home or in a care setting.
Deliver
time-specific, care-planned contact
alongside the patient's care coordinator.
Provide a range of services, including
practical and emotional support
, as outlined in individual care plans.
Work collaboratively with the MDT to ensure high-quality, person-centred care
