This post will be based in Driffield and covers Driffield, Hornsea and surrounding villages. Due to the geographical nature of the post it is essential that you can drive and have access to a car.



As a Community Mental Health Nurse you will assess and provide primary level interventions to promote mental health recovery with working aged and older adults. Establishing therapeutic relationships with service users and their family / carers.



Screening and initial assessment of needs.



Develop and deliver recovery focused care in collaboration with service users and their family / carers /residential settings.



Provide information and psycho education surrounding mental health conditions and medication. This may be 1:1 or group work.



Uphold legal frameworks which underpin service delivery.



Assess risks, the management of risks and effectiveness of interventions.



Signpost / referral to external services.



Develop good working relationships with neighbouring clinical teams and voluntary agencies to enable effective co-working and responsive transfers of care if necessary.
