Community Mental Health Practitioner: Access Pathway

Job Description

The Mental Health Practitioners have a crucial role in processing, assessing and determine the mental health and social care needs of a person referred to determine their ongoing needs and facilitation of service provision. The role will require excellent risk assessment and management skills in relation to and positive risk taking and decision making to enable safe and effective responses for an identified client group, largely by telephone/assessment.



You will participate in the delivery of the agreed aims and objectives of the service and identify factors which may inhibit these from being achieved. They will assist in the delivery of a high standard of Mental Health care to service users referred to the service. They will implement agreed systems for the monitoring of effectiveness of care.



You will be based with either the Shropshire or Telford Crisis Resolution Home Treatment Team (dependant on preference). At times, there will be some expectation to be based at either The Redwoods Centre or Hall Court.



You will be working closely with the Crisis team and the Access Call handlers.



The post is working 24 hours per week



Shift pattern:

Monday to Friday 4pm - 9pm- 15 minute break



Leading on the assessment of people referred to the Access Team.



Development and maintenance of effective communications within the area of work and promotion of positive working relationships with pathway colleagues, departments and other agencies.



Organising and delivering informal, team-based, training to the wider team which focuses on the particular needs of all age clients with functional mental health problems.



Managing time effectively to ensure the best use of staff and resources when acting as Shift Co, informing senior members of the team/pathway when issues impacting on service delivery become apparent.
