We have a full-time, permanent Band 6 vacancy for a Triage practitioner based at Hill Rise in Newhaven.



Newhaven is a fishing port set in the beautiful South Downs National Park and within easy travelling distance from Brighton or Eastbourne.



You would join our Triage team, which also includes two Band 7 Senior Nurse Practitioners, a Support Worker and two dedicated Admin colleagues . The team manages all the new referrals to the Neighbourhood Mental Health teams and they work closely with our colleagues in primary care, the voluntary sector and NHS Talking Therapies to ensure people needing mental health support get to the right clinician at the right time, according to their level of need.



You will need to have experience of assessing mental health needs. You will need excellent interpersonal skills for this front facing role and have the ability to work efficiently but also with a meticulous eye for detail.

1. To work predominantly in Triage screening new referrals, clinical decision making and liaising with referrers/ other partner agencies



2. To undertake initial assessments of individuals and their families



3. To complete risk assessments and develop care plans



4. To contribute to risk management and safety planning



5. To ensure that clinical practice is evidence based and consistent with relevant NICE guidelines and Trust policies and procedures.



To provide supervision of junior staff and trainees where appropriate
