We are seeking a confident and compassionate Physiotherapist to join our growing community rehab team, delivering patient-centred care across clinics, home visits, and care homes.
This dynamic role offers the opportunity to make a real impact through rehabilitation, falls prevention, and respiratory care, while contributing to service development and quality improvement initiatives. You'll work closely with a multidisciplinary team--including GPs, OTs, nurses, and social care professionals--to deliver joined-up care, and engage in MDT meetings to support effective rehab planning. The role also involves educating care home staff and families, mentoring junior team members, contributing to in-service training, and supporting the expansion of care home physiotherapy services. With access to CPD events and opportunities to shape evolving models of care, this is a flexible, varied position ideal for someone passionate about empowering patients and improving outcomes.
Responsibilities
Delivering physiotherapy to older adults in the community, recovering from illness, managing long-term conditions or looking to maintain independence.
Neurological rehabilitation - including stroke, Parkinson's, MS, and acquired brain injuries
Falls prevention and functional rehabilitation for our elderly clients
Training care staff to level up the care they provide.
Learning disability support - optional involvement with our PMLD services
Respiratory Care - optional input into developing your community respiratory skills with our specialist physiotherapist.
Collaborative working with MDTs and the NHS - supporting patient flow and discharge planning
Requirements:
BSc/MSc in Physiotherapy and HCPC registration.
Minimum 2+ years' experience, ideally but not essential with some in elderly / community / neuro rehabilitation
Holds a valid driving licence and has access to a car for community visits
Strong MDT working and patient-centred approach.
A passion for rehabilitation, falls prevention, and optimising patient flow.
