Responsibilities:
Conduct comprehensive assessments and develop individualized care plans to meet patients' clinical needs
Administer medications, injections, and treatments in line with clinical guidelines and prescriptions
Monitor, evaluate, and record patients' health conditions, escalating concerns appropriately
Provide wound care, catheter care, palliative support, and other community nursing interventions
Educate and support patients and their families in managing health conditions and treatment plans
Collaborate with GPs, allied health professionals, and social care teams to ensure integrated care delivery
Maintain accurate, confidential, and compliant patient records in line with regulatory and professional standards
Promote independence, dignity, and quality of life for patients in their own homes or community settings
Qualifications:
Registered Nurse
(NMC Pin required)
Previous experience in community or primary care nursing is desirable
Strong clinical assessment and decision-making skills
Excellent communication and interpersonal abilities
Ability to work independently as well as part of a multidisciplinary team
Full UK driving licence (preferred for community-based travel)
Nice-to-Have Skills:
Experience in chronic disease management, wound care, or palliative care
Familiarity with electronic patient record systems
Confidence working in diverse cultural and multilingual communities
