Job Description

The post holder will be welcomed as a member of the friendly Community Learning Disability Team in the West of Northumberland.



As an integral part of Northumberland community learning disability team, the post holder will work into the community with families and carers and care providers. They will be responsible for an individual caseload, they will work across primary and secondary health care services to ensure equality of access for those with a learning disability. The post holder will assess, plan, implement and evaluate high quality of care for adults with learning disability.



The post holder will be offered robust support and supervision, opportunity to attend regular team meetings with the wider team and will be offered opportunities for continued professional development.



The post will be based within the Northumberland Community Learning Disability Team, in west locality.



The post holder will manage their own case load, complete physical health assessments and care plans. Creating risk assessments supporting induvial with mental health needs to access primary and secondary care. They will also be involved in the development of specialist positive behavioural support plans for clients and service providers in order to reduce of prevent placement breakdown.



The post holder will support the skills of the wider CLDT service to ensure physical health and wellbeing is prioritises and embedded in aspects of specialist work.



They will be expected to chair, attend and contribute to all relevant meetings. Provide supervision to junior staff. Be involved in developing and delivering training and partake in group work.
