An exciting opportunity has arisen for a passionate and enthusiastic qualified nurse to work within the Sunderland West Community Treatment Team. The successful candidate will be joining a dynamic Multi-Disciplinary Team at an exciting time of positive service transformation.
The post holder will be expected to demonstrate high level of commitment, motivation and knowledge of working with people with severe and enduring mental health problems. The role will require clinically effective assessment, care management skills and interventions.
The post holders duties will include engagement with clients in the development of person centred, recovery focused care. You will need to demonstrate excellent communication skills and be able to work collaboratively in a multidisciplinary setting.
You will be dedicated to delivery, both as part of a team and working independently, of a high-quality, innovative, recovery focused and person centred care which meets the needs of people who use our service and their carers.

Main duties of the job

As a core member of the team, you will also be responsible for your own caseload, providing ongoing assessment, agreeing collaborative care plans as a Key Worker and delivering evidence based intervention in the community.
Your role will involve work within the teams physical treatment pathway, which provides medication management, monitoring and administration.
You will have an enthusiastic approach towards your own personal and professional development. In return, you will receive regular appraisal, clinical, management and caseload supervision and will benefit from the support of an experienced and committed multi-disciplinary leadership team.

• Experience of working with service users and carers in successful delivery of treatment plans
• Multi-disciplinary team work.

Desirable

• Experience of physical treatment pathways in community mental haelth services

Knowledge and Experience
