We are looking for an enthusiastic, dynamic, Band 5 Nurse to join our Intensive Support Team (IST) for adults with learning disabilities, in Peterborough. This team offers a unique opportunity to show case your experience and expertise in supporting adults with learning disabilities in a crisis. The successful applicant must have NMC Registration as a Learning Disability Nurse (RNLD) or Mental Health Nurse (RNMH)
The successful applicant will possess practical clinical skills; have lived experience of supporting a person with learning disabilities with behaviours of concern and communication needs. Experience of working as part of a team; undertaking holistic health care, be capable of providing accurate report writing, and understand legislation that guide practice.
You will be joining a team of motivated health professionals where we value multi-disciplinary working and continually strive to develop our skills and knowledge to meet the needs of service users in a creative holistic way using a Positive Behavioural Support (PBS) approach.
IST supports adults with learning disabilities with a diverse, range of complex needs. Our goal is to support our service users to remain in the community at a time or crisis, by offering additional mental health support, therapy, and treatment. We support the transition of service users to return from out of area placements and work closely with services supporting young people to prevent them leaving Peterborough.
The main role of a community nurse in IST is to triage and manage crisis referrals and participate in the delivery of specialist Learning Disability nursing under the guidance of a senior Nurse.
The nurse will participate in fortnightly multi-disciplinary team meetings and feedback on their caseload, they will attend other multi agency meeting within CPFT and with Key partners.
The role will include collaborative working appropriate information sharing with family, Carers, Police, and Probation, GP and other key stakeholders and partner agencies.
The role will involve participating in the writing of Positive Behaviour Support (PBS) plans, and risk formulations with other team members.
Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licenceand have use of a vehicle. You cannot use public transport for this role as this is not a reliable form of transport and will not allow you to meet service needs. The nursing role is to offer specialist support/ interventions to service users with complex needs and/or behaviours that challenge to prevent admission to hospital or to out of area placements.
To Participate in Care Treatment Reviews(CTR) for Service Users in hospital out of area or locally in assessment and treatment services or for individuals in the community.
To complete the dynamic Support Tool so that service users can re rated on the Dynamic Support Register (DSR) to show levels of support needed on rage a rated system.
To regularly monitor and review medication changes with psychiatry.
To ensure that clinical practice is evidence based and consistent with relevant NICE guidelines and Trust policies and procedure
