Job Description

An exciting opportunity has arisen for a Band 5 Community Nurse based in Ceredigion with Lampeter/Llanybydder District Nursing team.



The role will allow you to support the management of a delegated caseload within the Community Nursing Team providing high quality and complex care management to patients in their own homes, with the guidance and support of an experienced District Nurse Team Leader.



The post holder will ensure high standards of care are delivered to patients and their families/carers within the home or clinic environment. This includes liaising with staff, patient/carers and other agencies/organisations as required to ensure appropriate delegation of duties and risk management, whilst ensuring effective communication between those involved in the care package.



Pembrokeshire's Community Nursing Teams have a wide-ranging nursing skill mix and pride themselves on supporting professional development and improving high standards of community practice in line with current health policy.



This post is worked over a 7-day period including weekends and bank holidays.



The post holder will need the ability to travel between sites in a timely manner.

You will:

Be flexible and adaptable in order to accommodate the needs of the service, be able to contribute to the performance of the community nursing team and develop professionally within the role.



Assess nursing needs, deciding on the use of appropriate assessment tools, to plan appropriate nursing care, deciding on frequency of visits, deciding when care should be reviewed, altering care plan accordingly, in conjunction with the District Nurse Team Leader. This will ensure successful outcome for appropriate care designed and adapted to meet individual need. All care provision based on robust evidence based clinical research.



Work alone and with confidence to make decisions without supervision.



Work in line with the Health Board Lone Worker policy.



This is a fixed term/secondment vacancy until 01/08/2027 due to funding.
