We are looking for a caring, motivated and dynamic Community Nurse to join our friendly, supportive team. This is a great opportunity for someone who is confident working autonomously while still valuing strong team collaboration.



You will be part of the Toressa Community Nursing Team, working across Penntorr Health Centre and St Barnabas Hospital, covering Torpoint, Saltash and the surrounding areas. Our team works with local GP practices (Rame Group, Quay Lane, Port View and Saltash) as well as health and social care colleagues, therapists and voluntary sector partners. We're looking for someone who brings enthusiasm for community nursing, is committed to person-centred care and is keen to contribute to innovative ways of meeting the changing needs of our community.



We welcome applications from nurses who are proactive, adaptable and confident in assessing and managing care in patients' homes. An interest in working closely with patients, carers and wider MDT partners is essential, along with a commitment to continuous professional development.



The role involves regular travel to patient homes and community sites, along with the transportation of essential clinical equipment. Given the limited public transport options in Cornwall, candidates must have access to appropriate transport that can be used for work duties.




Main duties of the job




Shift patterns are 08:00-16:00 and 14:00-22:00.



Following an initial induction and ongoing clinical supervision, you will be responsible for: Assessment, planning, implementation and evaluation of care, without direct supervision, to patients, carers and relatives within their home environment. The aim is to enable them to enjoy maximum independence and optimum quality of life.



To work in partnership with patients and carers to offer support and advice and promote informed choice.



To work autonomously, providing effective, evidence-based care in line with competency and training record and organisational policies, procedures and guidelines.



To support all locality teams by working in a flexible way. Moving to work with the team in most need on a daily, weekly or monthly basis as service provision demands.

Essential criteria




Registered Nurse (Adult) Evidence of recent relevant professional development To act as a mentor for student university placements. Having the mentoring qualification or being willing to work towards this qualification and attending mentorship updates as required. Current Professional Registration - NMC

Hold a post-registration diploma or be working towards a degree #

Essential criteria




IT skills to include use of Word, Excel, email, internet, recording systems The ability to provide skilled, evidence based nursing care to a wide range of patients Ability to deal with patients and carers who are under stress. Awareness of and adherence to policies including lone worker and manual handling policies Current knowledge of developments in nursing and healthcare in general.
