Job Summary

We are seeking a dedicated and compassionate Community Nurse to join our healthcare team. The Community Nurse will play a vital role in providing high-quality nursing care to individuals within the community hospitals. This position requires excellent communication skills, strong leadership abilities, and effective time management to ensure the delivery of personalised care plans that meet the diverse needs of our patients.

Responsibilities

Deliver comprehensive nursing care to patients in their homes or community settings, ensuring adherence to care plans. Assess patients' health status and develop individualised care plans in collaboration with multidisciplinary teams. Communicate effectively with patients, families, and healthcare professionals to provide updates on patient progress and address any concerns. Educate patients and their families about health management, medication administration, and lifestyle modifications. Maintain accurate patient records and documentation in compliance with regulatory standards. Demonstrate leadership by mentoring junior nursing staff and supporting their professional development. Manage time efficiently to prioritise patient needs while maintaining a high standard of care.

Qualifications

Registered Nurse (RN) qualification with a valid nursing licence. Previous experience in community nursing or a similar role is preferred but not essential. Strong communication skills to effectively interact with patients and healthcare teams. Leadership qualities that inspire confidence and promote teamwork within the nursing staff. Excellent time management skills to handle multiple patient cases efficiently. Ability to develop and implement effective care plans tailored to individual patient needs.
