Job Description

We have an exciting new position for an experienced Occupational Therapist to join the long-term conditions services in the North Lancashire area . The role will be embedded within the C are H ome S upport & L ong T erm C onditions team, providing a multi-disciplinary approach to patient centred care. The post holder will work closely with the Advanced Physiotherapist within the Care Home Support & Long Ter m Conditions Team , and wi th therapy colleagues across the wider Community T herapy team , including profession specific clinical supervision and support.



The Care Home Support Team support staff in registered Nursing homes in the North Lancashire footprint to deliver safe, high-quality care for their residents.



The Long-Term Conditions (LTC)Team plays a significant part in caring for people, who have been assessed as having a long-term condition or who are frail and whose care is more appropriately delivered at home.



All these teams work towards admission avoid ance and early discharge from hospital, following the Government plan to prevent, delay or divert the need for acute hospital or long-term bed-based care.



The post holder will work independently as an autonomous practitioner managing a caseload of patients with a range of frequently complex conditions, appropriate to specialist knowledge and skill level and experience; providing flexible and patient centred programmes of Occupational Therapy (OT) interventions.



The post holder will take responsibility for the management of their own caseload without direct supervision, whilst continuing to develop specialist clinical skills, knowledge and experience as identified in the post holder's Performance and Development Plan (PDP).



The post holder will assess, diagnose, plan, implement and evaluate individualised programmes of evidence-based care providing highly specialist advice to patients, carers/relatives, and the multidisciplinary team.



We operate from three main hospitals - Furness General Hospital (FGH) in Barrow, the Royal Lancaster Infirmary (RLI), and Westmorland General Hospital (WGH) in Kendal, as well as a number of community healthcare premises including Millom Hospital and GP Practice, Queen Victoria Hospital in Morecambe, and Ulverston Community Health Centre.

FGH and the RLI have a range of General Hospital services, with full Emergency Departments, Critical/Coronary Care units and various Consultant-led services.



WGH provides a range of General Hospital services, together with an Urgent Treatment Centre, that can help with a range of non-life threatening conditions such as broken bones and minor illnesses.



Our community services for adults are provided in people's homes, community centres, clinics, GP Practices, community hospitals and our main hospitals. Our aim is to work with people to help them remain independent, improve their health and manage their conditions through high quality care, advice and support.



The team delivers a 5 day service Monday to Friday. You will be expected to be able to travel across south Cumbria and North Lancashire and be flexible for the needs of the service with some expectation to work across traditional boundaries. A broad experience of delivering rehabilitation is advantageous.
