About the Role



We are recruiting Community Outcomes Facilitators to support adults accessing Daytime Opportunities across Tameside.

This is not a traditional support worker role. You will act as a facilitator, mentor and enabler, working alongside individuals to help them build confidence, independence, social connections and meaningful community lives.

The role is aligned to Tameside Council's Daytime Opportunities Flexible Purchasing System, focusing on person-centred, strengths-based and outcome-led support.

What You'll Be Doing



Supporting adults to achieve personal goals, aspirations and outcomes Facilitating group and individual daytime activities in community settings Encouraging independence, choice and positive risk-taking Supporting access to community, education, volunteering, employment and social opportunities Building strong, respectful relationships based on trust and equality Promoting social inclusion, friendships and community participation Supporting engagement in digital and innovative opportunities Working in partnership with families, carers, colleagues and local professionals Following safeguarding procedures and maintaining accurate records

Essential Experience



Experience supporting adults with: Learning disabilities Autism / neurodiversity Mental health needs Physical or sensory needs Understanding of person-centred and strengths-based practice Willingness to work flexibly across Tameside

Training & Compliance



You'll receive a full induction and ongoing training, including:

Safeguarding Adults & Children Mental Capacity Act Autism Awareness Oliver McGowan Mandatory Training Equality & Diversity First Aid & Health & Safety
Enhanced DBS required (or willingness to obtain).

You will



Facilitate 1:1 and small group sessions (3, 5, or 7 hours) Promote independence, confidence, and positive risk-taking Encourage people to develop new skills (e.g., travel, daily living, social skills) Support people to build friendships, explore interests, and improve wellbeing Keep clear, accurate records and contribute to support plans Ensure safeguarding, safety, and dignity always

About You

Essential:



Experience in care, support, education, community work, or similar Enhanced DBS (or willingness to obtain one) Right to work in the UK Driving licence and access to a vehicle NVQ Level 2/3 Health & Social Care (or willingness to complete)

Hours



Flexible full and part-time hours
Session lengths:

3 hours, 5 hours, or 7 hours


Weekdays, with weekends depending on demand.
