As part of our continued service development, we are delighted to offer an exciting opportunity for a Band 6 Physiotherapist to join our dynamic Intermediate Care Team. This role will support all aspects of the service, including Intermediate Care beds across the King's Lynn area.
Our multidisciplinary Therapy Team is committed to delivering high-quality, evidence-based rehabilitation both in Intermediate Care settings and in patients' own homes. We focus on enabling early supported discharge and promoting patient independence.
We are seeking a motivated and proactive physiotherapist who can demonstrate excellent communication and interpersonal skills, sound clinical reasoning, and the ability to adapt in a fast-paced and evolving environment. Ideally, the successful candidate will have previous experience in discharge planning and in developing robust rehabilitation programmes within a community or intermediate care setting.
The post-holder will be responsible for conducting therapy assessments for patients in rehabilitation environments. Depending on patient needs, this may include both planned and unplanned community visits. The role also includes the supervision of Band 4 Assistant Practitioners, and therefore requires a strong team player who is confident in supporting and guiding junior staff.
Main Duties & Responsibilities
Key Areas
1. To assess and prescribe treatment packages for patients with long term conditions, the frail & elderly, palliative care and rehabilitation needs for example, to achieve quality of life and independence where possible.
2. To work within the designated team to facilitate early discharge from hospital
3. To work within the designated team to prevent unnecessary admission to hospital
4. To provide in-reach to Intermediate Care Beds
5. To work with all health care professionals, and statutory/non statutory agencies to provide a seamless, integrated service to our service users.
Clinical supervision will be provided by a Band 7 Clinical Lead Physiotherapist. You will be working within an experienced, supportive, and forward-thinking team that values collaboration and offers ongoing opportunities for professional growth and development.

The right candidate will be required to travel and work within the Kings Lynn area where the Intermediate Care beds are located.
