We have an exciting opportunity within the Homefirst and Community Rehabilitation teams, part of the Penwith Integrated Community Team. Our multidisciplinary team includes nursing, therapy, and admin staff who work closely with partners to support rehabilitation needs across the local area.
We are looking for a physiotherapist with community experience who is eager to develop their assessment, care planning, and rehabilitation skills in a dynamic, supportive, and research-active environment. You'll take the lead in assessing patients, setting goals, and supporting reablement across a varied caseload, primarily involving older adults.
We offer a supportive working culture, structured training, regular supervision, and professional development. The team is based in Hayle and Penzance and covers East and West Penwith PCN and the Isles of Scilly.
Up to 75 hours are available--we welcome applications from those seeking full or part-time roles. The service operates 8am-6pm, 7 days a week, including some weekends and bank holidays, so flexibility is essential.

You will need to be able to a triage referrals to identify need and urgency, assess and provide highly specialist evidence based treatment plans for adults within the catchment area, either in their own homes or care homes.
To work alongside the wider multidisciplinary teams within the community which includes partner agencies, liaising with the community and acute hospitals, Adult Social Care and the Voluntary Services
