More detail about the role Come and join the Community Practice Education Team, who work across the whole of Eastern, Mid and Northern Devon services, making a difference to those who deliver, and receive care in a variety of settings. No two days are the same, where you can develop and apply a wide variety of clinical skills and knowledge as a clinician. We work closely with wider services such as Safeguarding, QAIT, Devon County Council, CQC, ICB and many more, to be assured escalated concerns are being addressed promptly, to make services safer for all our community. The Community Practice Educator will be working in the Community Practice Education Team, with close partnership with Care Services, Voluntary Services and Community Teams to; Deliver proactive education in a wide range of topics across community teams, care homes and external organisations in line with the Enhancing Health in Care Homes Framework (EHCH) (NHS), and local organisational objectives. Be involved in a wide range of topics & projects within the Community Services and Care Services. Develop and build effective working relationships with key agencies; identifying those who would benefit from education in a wide range of topics to upskill their staff.
Support Community colleagues to reduce hospital admissions from the community, by advocating high quality care skills, escalation and multi-professional conversation, advice, education and signposting. To collate, analyse and present data in relation to admission avoidance and effectiveness of the topics delivered. Create, update, review and deliver education packages, and bespoke learning for external care settings & providers, as well as community teams within the Royal Devon. These conversations may be tough at times. You will be working with wider services also, to ensure integrated working across Devon with our wider service colleagues.

There is opportunity to develop yourself as a clinical educator through a wide range of programmes/training available. Candidates will need to own their own car, with business insurance as the role involves driving to visit settings and teams across Eastern, Mid and Northern Devon.
