Job Description

An exciting opportunity has arisen in the Older Persons Community Treatment Team Newcastle/North Tyneside, for a Band 6 Community Practitioner (Nurse, Social Worker or Allied Health Professional) based in the Centre For Aging and Vitality (CAV) on the old Newcastle General Hospital Site.
We are seeking a highly motivated clinician to undertake assessment and offer treatments. Experience in working with older adults is required. The post holder will provide assessment and treatment function, this would be across a variety of environments e.g. community care home or patient's own accommodation. The post holder will be a Care Coordinator working not only with the Mutli Disciplinary Team but also working with other agencies and voluntary sector
You will possess excellent interpersonal and communication skills. You will have the ability to work both independently and within a team. You will demonstrate the ability to liaise with different agencies. You will need to be skilled and comfortable in dealing with clients who have multiple and complex needs, including issues around risk management. This post holder will be required to work Monday to Friday.
Main duties of the job
The successful candidates will have responsibility for a shared caseload working within and supported by a skilled and experienced MDT to deliver safe, effective, responsive and compassionate care to our service users.You will ideally have clinical experience in assessing, formulating and managing risk and complexity within older persons mental health/learning disability services. You will be able to develop your skills by providing care and interventions to service users with a range of complex mental health needs with associated risk.
You should have a collaborative and enthusiastic approach as well as the passion and commitment to developing and promoting recovery-based approaches for older persons with mental health related needs.

  • Professional Qualification and current Professional Registration as, a First Level Registered Nurse (RNMH, RMN, RNLD) or equivalent as an AHP, Social Worker or Clinical Psychologist
  • Evidence of post-registration training in clinical/ psychological interventions / skills relevant to the post.
  • Up-to-date knowledge/experience of relevant legislation.
  • Proven track record of post registration clinical experience relevant to the post.
  • Mentor Training.
Desirable
  • Post Registration Qualifications in Psychological Interventions
