We are looking for a skilled and motivated Band 6 Community Practitioner to join the Newcastle and Gateshead Step Up Hub based at Silverdale. This post offers a dynamic and rewarding mix of community and hospital in-reach responsibilities, with a focus on delivering high-quality, rehabilitation-oriented care. The successful candidate will work with a small caseload of service users who may require an assertive approach and/or community rehabilitation.
In this role, you will provide care coordination and nursing support to service users with complex mental health needs across both community and inpatient settings. You'll play a crucial part in ensuring smooth transitions between hospital and community care, attending Mental Health Act tribunals, preparing professional reports, and working closely with multidisciplinary teams.
You will need to be clinically experienced, solution focussed and competent in working with individuals, their families, and carers, in a community setting. You should have a collaborative and enthusiastic approach as well as the passion and commitment to develop and promote recovery-based interventions for adults with mental health related needs.

Deliver high-quality, person-centred mental health care in both community services
Undertake comprehensive risk assessments and care planning
Attend Mental Health Act tribunals and professionals' meetings as required
Prepare formal clinical reports for tribunals
Work collaboratively with inpatient teams to facilitate discharge planning and continuity of care
Provide support and input during mental health crises and implement robust risk management plans
Mentor and support junior staff, students, and colleagues
Liaise effectively with GPs, social workers, carers, and other service
