Job Description

The Community Treatment Team provides comprehensive, person-centred support to adults experiencing complex or enduring mental health difficulties. Working as part of a dynamic, multi-disciplinary team, the post holder will contribute to the delivery of high-quality, recovery-focused care within the community.



The role involves working collaboratively with service users, their families, and other health and social care professionals to promote mental wellbeing, independence, and social inclusion. The post holder will undertake assessments, develop and implement care plans, monitor progress, and respond to changing needs, ensuring that all interventions are safe, effective, and aligned with evidence-based practice.



This position requires strong interpersonal skills, professional judgement, and a compassionate approach to supporting individuals to achieve their personal recovery goals. The successful candidate will also contribute to service development, risk management, and the ongoing improvement of community-based mental health care.



Deliver high-quality, person-centred mental health care in community services



Undertake comprehensive risk assessments and care planning



Attend Mental Health Act tribunals and professionals' meetings as required



Prepare formal clinical reports for tribunals



Provide support and input during mental health crises and implement robust risk management plans



Mentor and support junior staff, students, and colleagues



Liaise effectively with GPs, social workers, carers, and other service
