Job Purpose:

To deliver holistic, evidence-based care in a community setting as part of a multi-disciplinary, multi-agency team. The post holder manages a defined caseload, performs comprehensive assessments, develops and reviews care plans, and acts as Care Coordinator under the Mental Health Measure

Main Responsibilities

Provide assessment, care, and treatment to adults and older adults with mental-health problems. Conduct risk assessments and crisis interventions. Administer oral/intramuscular medications; monitor physical health. Work with partner agencies - Local Authority, Primary Care, and voluntary sectors. Support carers, complete carer assessments, and contribute to discharge and after-care plans. Maintain accurate clinical documentation (NMC standards). Supervise junior staff and mentor students. Participate in audits and service-development initiatives.

Essential Qualifications & Experience

Registered Mental Health Nurse (RMN). Strong knowledge of the Mental Health Act, Mental Capacity Act, and Mental Health Measure. Clinical experience in adult or older-persons mental-health services. Ability to assess, formulate, and coordinate complex care plans. Excellent communication, IT, and organizational skills.

Desirable:

Welsh-language ability; prior community-mental-health experience.

Pay: 39,000.00-47,000.00 per year
