Are you passionate about helping people live independently in their own homes? We're looking for a proactive and compassionate Community Reablement Coordinator to join our Out of Hours team. This is a vital role supporting residents of Reading by coordinating care packages and working collaboratively with health and social care professionals to deliver person-centred support.

About The Role:



You will be responsible for receiving, triaging, and implementing referrals into the service, ensuring timely and effective care delivery. Using various IT systems, you'll administer referrals, optimise staff capacity, and minimise reliance on agency workers.



You'll liaise with service users and staff to coordinate care calls, deliver packages that meet individual goals, and maintain accurate and confidential records. Your role also includes responding to informal complaints, identifying training needs, and ensuring compliance with safeguarding, CQC, and health and safety standards.



In addition, you'll build strong relationships with internal and external stakeholders, support staff wellbeing, and act as a point of contact for Charles Clore Court staff outside core hours. You'll contribute to service development, promote equality and diversity, and uphold the Council's values through collaborative working and continuous improvement. Flexibility, creativity, and a commitment to person-centred care are essential to succeed in this role.



Working Pattern (3-Week Rotation)



We operate a rotating 3-week shift pattern designed to offer a healthy work-life balance:


Week 1:

+ Monday to Wednesday: 5:00 PM - 11:00 PM + Sunday: 6:45 AM - 10:45 PM
+

Total: 34 hours


Week 2:

+ Thursday: 5:00 PM - 11:00 PM + Friday: 4:30 PM - 11:00 PM
+ Saturday: 6:45 AM - 10:45 PM
+

Total: 28.5 hours


Week 3:

+

Full week off


This pattern provides regular time off while maintaining a consistent and predictable schedule.



About You:



You'll have experience in adult social care, reablement, or a similar setting, with a good understanding of how to promote independence and wellbeing. You'll be confident making quick, informed decisions and able to stay calm under pressure. Strong communication, organisational and IT skills are essential, along with a flexible, hands-on approach.



You'll also need the ability to work independently during out-of-hours shifts and as part of a wider multidisciplinary team. A caring, professional attitude and a commitment to providing the best outcomes for residents are key to success in this role.
