We are seeking innovative and motivated Registered Nurses to join our Camden Integrated Neighbourhood Teams . You'll play a vital role in delivering nursing interventions to housebound patients, supported by a skilled and collaborative multidisciplinary team.

As a Band 5 Community Nurse within the Camden Integrated Neighbourhood Team, you will:



Provide high-quality, evidence-based care to patients in their own homes or community settings.



Manage a designated caseload, conducting holistic assessments and developing individualised care plans.



Monitor and review patient care, adjusting interventions based on clinical need and best practice.



Work autonomously while maintaining strong communication with your team and other professionals.



Collaborate with a multidisciplinary team, including Complex Care Nurses, District Nurses, Occupational Therapists, Physiotherapists, Pharmacy Technicians, and Social Care partners.



Provide clinical supervision and support to junior staff and students to promote learning and safe practice.



Liaise with specialist teams such as Palliative Care, Tissue Viability, Bladder & Bowel, Rapid Response, Respiratory, Heart Failure, Stroke and Neurology, and Diabetes Teams.



Maintain accurate and timely records using SystmOne.



Participate in clinical supervision, team meetings, and quality improvement initiatives.



Be flexible in working across all five Camden Neighbourhood Teams to support safe staffing and skill mix.

Band 5 nurses are vital members of our community teams and play an integral role in how people experience care. Your role involves providing compassionate, safe, and responsive care to patients while maintaining professionalism and contributing to service improvement.



Key Responsibilities:



Deliver patient care in line with clinical guidelines, trust policies, and NMC Code of Conduct.



Recognise and act on safeguarding concerns, and escalate appropriately to protect vulnerable individuals.



Act as a patient advocate, supporting shared decision-making and informed consent.



Supervise and support less experienced staff and students, ensuring safe and high-standard care delivery.



Engage in reflective practice and promote a culture of continuous improvement and learning.



Maintain comprehensive and accurate records of care interventions and assessments.



Remain up to date with mandatory training, professional development, and revalidation requirements.



Clinical Skills You'll Be Supported to Develop:



Wound care & tissue viability :

Leg ulcers, pressure ulcers, VAC therapy, surgical wounds, doppler assessments.



Diabetes management :

Blood sugar monitoring, insulin administration, liaison with specialist diabetes nurses.



Bladder & bowel care :

Catheterisation (male, female, suprapubic), continence assessments, bowel care.



IV therapy :

Medication administration, care of PICC/midlines.



Palliative care :

End-of-life care planning, symptom control, syringe drivers, anticipatory medication.



Drain care :

Management of rocket drains, nephrostomies, chest drains.



Multidisciplinary and Integrated Working:



You will work closely with:



Camden's community nursing and therapy teams



Evening and Overnight Nursing Service



Urgent Community Response Team



In-patient rehab services and specialist medical teams



GPs, Social Care, Mental Health Services, and Voluntary Sector Partners



Support and Development:



Comprehensive preceptorship programme for newly qualified and community-transitioning nurses



Regular clinical and managerial supervision



Dedicated Nurse Development Lead to support your progression



Access to Clinical Practice Educators and in-house training opportunities



Involvement in research, audit, and quality improvement projects
