Job Description

The post holder will promote and support the health and wellbeing of all school-aged children, their families and the wider community.



The post holder will be expected to carry out assessments of needs with school-aged children, young people and their families. The post holder will also work on the implementation of planned programmes of care delegated by the School Nurse specialist practitioner.



CLINICAL RESPONSIBILITIES



Contribute to the identification and protection of children at risk of abuse and act in accordance with the local Safeguarding Children's Board guidelines. To attend safeguarding and other meetings to improve the health outcomes of children and young people as directed by the School Nurse Specialist Practitioner and provide written reports.



Assess and undertake selective health care interventions as directed by the School Nurse specialist practitioner, refer to other agencies/Health Care professionals as necessary.



Provide professional support and advice to children, young people, their families and schools and liaise and refer in discussion with the School Nurse Specialist practitioner



SERVICE DELIVERY RESPONSIBILITIES



Support delivery of the 5 -19 Healthy Child Programme (DoH 2009) and National Childhood Measurement programme under the direction of the School Nurse Specialist practitioner.



Support the planning, assessment of need and delivery of the agreed provision of a school health service to each school under the direction of the School Nurse Specialist practitioner.



Work within the Nursing & Midwifery Council's (NMC) framework for practice. All community staff nurse should have knowledge and work in accordance with Trust policies and guidelines including clinical supervision.



Actively promote uptake of al school nurse contacts, and public health interventions, screening and immunisations/vaccinations.
