Job Description
ClinicalThe ability to make a judgement about a patients mental health or wellbeing needsand act appropriately to manage needs / risks. (For example, supporting self-careneeds by helping a patient to organise their prescribed medication into a dosettebox, or reporting a concern / risk to a qualified member of staff).To contribute to the provision of a safe clinical environment and promote the dignityof patients.To prioritise workload based upon the patient needs / service demands.Assess the health and wellbeing of people within the CMHT as directed by therelevant professional.To provide health promotion for service users in the borough CMHTs,understanding the relationship between mental health and physical health. To beable to enhance a patients overall wellbeing and promote parity of esteem.To support multi-disciplinary meetings and contribute to information sharing andclinical decisions regarding service users in the care of the CMHTs.To support patients whose health and wellbeing is at risk, acting upon and reporting concerns appropriately Please see attached detailed job description and main responsibilities - Community Support Worker
