Due to the development of our adult services, we are looking for a Support
Worker to join our growing team.
Dwellin Social Care is seeking compassionate and motivated individuals to
provide personalised, community-based support to young adults with mental
health needs and/or learning disabilities. Each individual we support has their
own tailored plan, and you will play a key role in helping them achieve their
goals, aspirations, and independence.
Key Responsibilities:
As a Support Worker, your responsibilities will include:
Deliver person-centred support in line with individual support plans.
? Assist young adults to live independently, including support with
? Maintaining their tenancy and managing their home.
? Attending medical appointments and managing health needs.
? Completing housing applications and accessing community resources.
? Achieving personal outcomes and aspirations.
? Provide practical help with domestic tasks, including cooking, cleaning,
and budgeting.
? Support individuals to access social, educational, and leisure
opportunities in the community.
? Work flexibly to meet the needs of individuals
? Encourage independence while promoting dignity, choice, and respect.

Pay: 12.21-12.50 per hour
