The role is similar to a Health Care Assistant in that you will be delivering great care and supporting reablement for people, mainly in their own homes.
You will be working as part of the HomeFirst Restormel team, covering Newquay, St Austell, Padstow, and surrounding areas. Travel costs for journeys during work time are supported and pool cars are also available for work use although availability cannot be guaranteed. Due to the rural location of Cornwall, public
transport will not be sufficient to get to patient visits. Ability to get to visits independently is essential for this role, usually you would drive and have access to your own vehicle to achieve this.
You will need to demonstrate good communication skills and a positive approach to people who may have communication difficulties or who have challenging health needs.
You will be required to work a rota that covers 7 days a week from 08:00 to 20:00 hours. Flexibility within this rota, and geographical location, will be important.
This is a lone worker post.

optimised within the community setting

reinforce self-care wherever possible.
The Reablement workers within each locality will form a part of an Integrated Care Pathway for older patients that involve a range of
services to prevent hospital admission and support discharge from hospital
The post holder will be expected to deliver packages of care to patients in their own home; this may also include Residential homes and nursing homes

We're an NHS community and mental health provider Trust based in Cornwall and the Isles of Scilly. We deliver community and hospital-based care to improve people's physical and mental health. We also provide specialist support to people with dementia or a learning disability.
We are a people organisation and people matter to us. As part of the team, you'll help support the health and wellbeing of the people who live and visit this beautiful part of the UK.
Over 4,000 people make up the Trust. This includes doctors, nurses, therapists, plus admin and support staff.
We work in people's homes, in community clinics and bases. Some staff work from one of our 13 community hospitals. Our aspiration is to have great people, provide great care, be a great place to work and a great partner.
Approximately 568,000 people live here. A third of people who live in Cornwall are supported by acute hospital services in Devon. As a result, we also work closely with our partners in Devon. In the summer, and during other holidays lots of people choose to visit the area. This increases the numbers of people who use our services.
