The opportunity



We have a number of full-time and/or part-time exciting opportunities, to join our established team of Community Wellbeing Champions delivering our service, Community Referral Social Prescribing. The opportunities available will be ideal for someone who has a proven track record in supporting people to be independent, who has a good knowledge of adult social care and can work confidently with other professionals whilst working in different settings such as Primary Care, Community Settings and Local Authority Buildings. Do you have a passion for finding practical ways to enhance the lives of people facing complex life situations? Are you a team player and enjoy face to face community engagement? Community Referral Social Prescribing is an established service across Bedfordshire, we seek individuals who are keen to embed this approach to supporting people in their local community to improve their health and wellbeing. Beds RCC has been commissioned to provide Social Prescribing across the county and we are currently seeking to recruit Community Wellbeing Champions to deliver this service in a number of locations across Bedfordshire

The role



The BedsRCC Social Prescribing service ensures that people with a range of non-medical needs affecting their health are cared for and included within their local communities, by connecting them with Good Neighbour and Village Care schemes and/or other local activities and services. You will work alongside health and care professionals in a locality and GP surgery setting to identify suitable referrals; and help service users to identify and achieve their goals by means of a personalised support plan. You will build relationships with local community groups, and build community capacity to meet identified gaps in provision.

Who we are looking for



The successful candidates will be able to demonstrate a range of interpersonal, team working and problem solving skills, as well as a high level of IT literacy Microsoft 365 packages, Case Management and SystmOne. You will have a strong commitment to improving the lives of vulnerable people and those with poor mental health and connect lonely and isolated people to lasting support. You will have previously helped people to address their life goals and needs.

You will be able to 'hit the ground running' and be able to manage multiple priorities effectively within a fast moving environment. You will have a full driving licence and be prepared to work in-person in clinical settings and community settings. There may be occasional weekends / evenings.
