To provide high-quality, person-centred care and support to individuals with complex health and social care needs, enabling them to live as independently and comfortably as possible. This includes assisting with personal care, clinical interventions (following training and supervision), and promoting dignity, choice, and wellbeing.
Key Responsibilities
1. Personal and Clinical Care
Support clients with complex medical conditions such as spinal injuries, neurological disorders, tracheostomies, PEG feeding, ventilator care, or acquired brain injuries.
Deliver personal care sensitively and respectfully, including washing, dressing, toileting, and grooming.
Administer medication or clinical procedures as per care plan and training, under supervision where required.
Monitor and record vital signs, symptoms, and changes in health status.
Assist with mobility, transfers, and use of specialist equipment (e.g., hoists, wheelchairs, suction machines).
2. Emotional and Social Support
Build trusting relationships with clients and their families.
Support emotional wellbeing, companionship, and social engagement.
Encourage independence and participation in meaningful activities.
3. Record Keeping and Communication
Accurately document all care activities, observations, and incidents in line with company policy.
Communicate effectively with multidisciplinary teams, including nurses, therapists, and family members.
Report concerns or changes in condition promptly to senior staff.
4. Professional Conduct and Training
Follow all safeguarding, health & safety, and infection control procedures.
Participate in regular supervision, mandatory training, and competency assessments.
Maintain confidentiality and uphold professional standards at all times.
