Role Overview
The Complex Care Nurse is responsible for providing specialist one-to-one nursing care to clients in community settings, including private homes, supported living environments, and local community facilities. The role includes clinical interventions, care planning, medication management, symptom monitoring, and ongoing support for clients and their families.
This position plays a vital part in delivering care packages with a strong emphasis on safety, dignity, independence, and holistic wellbeing.
Key Responsibilities
? Deliver community-based clinical care for clients with complex needs including:
Ventilation support (invasive/non-invasive)
Tracheostomy management
PEG/NG feeding and nutrition plans
Suctioning and oxygen therapy
Seizure, respiratory, neurological, and cardiac monitoring
? Develop, implement, and review individualised person-centred care plans
? Administer medications and oversee safe medication practices in line with NMC Standards of Proficiency
? Provide clinical assessment, monitoring, and timely escalation of deterioration using provider protocols
? Train, mentor, and support healthcare assistants and support workers delivering client care
? Conduct competency assessments for staff supporting complex care packages
? Maintain accurate clinical documentation and care records in compliance with GDPR and company governance
? Communicate effectively with multidisciplinary professionals including:
NHS consultants, GPs, community therapists, social care teams
Case managers and commissioning teams
? Support families and carers with education, reassurance, clinical guidance and emotional support
? Participate in on-call rotas if required
Required Skills & Competencies
Registered Nurse qualification (Adult, Paediatric or Learning Disability)
Active PIN with the NMC
Experience working with complex care clients in community or acute settings
Confidence in respiratory support, enteral feeding, tracheostomy and neurological care
Ability to work autonomously in the community
Strong clinical assessment and decision-making skills
Excellent communication and leadership skills
Commitment to safeguarding and high standards of care
Valid UK driving licence preferred
