Title: Children and Young Adult Complex Support Worker

:

The role of the Children and Young Adult Complex Support Worker works directly with Children and Young Adults, predominantly in their own home, who have a diverse range of complex continuing health care needs, including learning difficulties , autism, physical disabilities, and complex challenging behaviours. You will find yourself at times working in health and social care settings, for example, acute hospital admissions, planned respite in a local service or hospice, and out in the community to support the unique and distinct needs of the child and young adult you working with.

You will provide specialised support to a child or young adult with a long-term health condition and/or disabilities, focusing on personal care, clinical support with the outcome of enhancing their quality of life.You will also manage the wellbeing and behaviour's you will be assisting with the care plans, risk assessments, reviews and incident reporting. You will need to maintain professional relationships with the person you are working with , family and external health care professionals at all times.

You will need to be able to:

Adapt and act accordingly to situations

Maintain a high level of patience and emotional resilience

Be non-judgemental regardless of the person's needs and behaviours

Remain calm under pressure and when dealing with challenging situations

Highly experienced in working in a home environment with a person with specialised and complex health care needs.

Key Responsibilities:

1.

Personal Care

: Assist a child and young adults with daily living activities such as bathing, dressing and maintaining hygiene. This includes managing medication and mobility support.

2.

Clinical Support

: Provide specialised clinical care as needed, for example, Tracheostomy and Long Term ventilation Care; Gastrostomy and Enteral Feeding Management, including MicKey, G Tubes Mini Buttons.

3.

Emotional and Family Support

: Offer emotional support to children and young adults and their families, helping them to manage with the challenges of complex health conditions. This may involve respite care and connecting families with relevant support services.

4.

Care Planning

: Collaborate with healthcare professionals to develop and implement personalised care plans that address the specific complex health care needs and preferences of the children and young adults you work with.

5.

Social Engagement

: Promote social interactions and activities to reduce feelings of isolation, helping children and young adults we work with maintain connections with friends and community members.

To be considered for this role you will need:

A minimum of 3 years work experiences withing with children and young adults with specialist and complex continuing health care needs in a home setting. Essential

3 years references without gaps - Essential

A full driver's license - Preferable

An enhanced DBS check on the update serv cie as well as a clean criminal record

To have gained qualifications in relevant subject to health and social care as level 2 as a minimum.

Job Types: Full-time, Part-time, Permanent

Pay: 13.00-14.00 per hour

Expected hours: 20 - 40 per week
