We're looking for a Complex Case Pathway Coordinator to play a pivotal role in improving outcomes for adults experiencing self-neglect, particularly where non-engagement with health interventions is a key factor.
This post recognises the complexity of coordinating multi-agency responses and aims to strengthen collaboration between health partners across Bromley. You'll work closely with GPs, primary care teams, and other professionals to ensure that those at risk of self-neglect receive the right support, at the right time, from the right services.
Based within the Bromley Healthcare Safeguarding Service and line-managed within the Adult Safeguarding Team, you'll act as a bridge between agencies -- facilitating communication, arranging and minuting meetings, developing effective pathways, and helping to shape a more coordinated approach to care

The Complex Case Pathway Coordinator will lead in the coordination of self-neglect cases, which are being managed through the Complex Case Pathway/ Framework .
The role includes completing all administrative, information and organisational duties to ensure maximum outcomes from Complex Case Pathway meetings. The post-holder will be highly conscientious, organised, with excellent communication, planning and people skills in order to effectively manage the main duties and responsibilities set out below.
