Complex Discharge Coordinator

Job Description


The role is an integral part of the patient experience and journey in the hospital. The role acts as a main point of contact for patients and their family along with multiple partners to ensure the appropriate care plan is in place for discharge.The aim is to remove barriers prior to discharge, aiming home first using a strength based approach.This role is primarily for Northwick Park Hospital, but the employee will need to remain flexible to cover cross-site when neededMain duties of the jobBe responsible to optimise hospital bed utilisation and reduce the length of stay by ensuring the timely, safe transfer/discharge of patients to the most appropriate and agreed environment. This will include monitoring the transfer of patients to non-acute rehabilitation centres and facilitating delayed transfers of care.Instigate and co-ordinate meetings with patient, family/carers and community agents and facilitate joint working partnership between Primary and Secondary Care.

  • Degree qualification or proven ability of working to a degree level with relevant experience in the NHS/Social Services
  • Evidence of formal/informal management training e.g. disciplinary/grievance, recruitment and retention, managing absence, finance and budget management

  • Evidence of theoretical knowledge and practical experience in a wide variety of specialist issues such as social law, immigration and continuing care rights
KnowledgeEssential
  • Experience of discharge planning within the acute setting
  • Previous experience of working within a multidisciplinary team
  • Experience of working with older people and their specific needs
