Job Description

The postholder willbe responsible forensuring thatIportalentries are up to date,accurateand saved on to the patients' record. The post-holder will be expected to work autonomously but will have support from Senior Colleagues and the Lead Clinician. Responsible for holding multi-disciplinaryteammeetings, with key professionals(both primary & secondary care)whoare responsible forproviding servicestoensurerobustplansareinplaceto preventfuture admissions. Provide advice and support for practice nurses & community matrons in primary care in the management of complex patients.



The postholder will be expected to have excellent communication skills and be willing and able to communicate with the patients, medicalstaffingand the wider health economy. The post-holder will be key in mapping change in the Trustand the wider health economyin managing COPD patients. The post holder will need to be flexible & prepared to provide a7-dayservice as the needs of the service change. Although working autonomously, the post holder willbe requiredto have daily huddles with the team lead & wider COPD team to ensuregood communication& handover.
